General Eligibility Criteria for availing this scheme:
To be eligible in Gujarat Widow Assistance Yojana you have to meet the following eligibility criteria

Applicant’s age should be 60 years or above anywhere.

If you are applying for old age pension scheme, you will need the following documents.

Proof of income (must have income as above)
2 passport size photographs
Birth Certificate (Example of Date of Birth)
Copy of Ration Card
Bank or Post Passbook
Mobile number
Certificate of not having a son of 21 years (Talati cum Mantri)
Life certificate has to be submitted every year.
Should not have son above 21 years of age. Having son but physical, mental disability
Having cancer, t. If V is suffering from serious illness to get benefit of this scheme

Fill the application form
Attach all the above documents.
Submit this form to the Social Security office.
Then, you will get approval certificate from Mamlatdar department.
(a) Who can benefit?
Elderly aged 60 years or above.
A member of the family named in the poverty line list with a score of 0 to 20
(b) Place of submission of application
Certificate of Name on Poverty Line List.

(d) Benefits payable under the Scheme
Beneficiary aged 60 to 79 years will get Rs. Rs. 1000/- and beneficiary aged 80 or above Rs. 1250/- monthly allowance is paid.
(e) Payment of Assistance
Payment is made through DBT into the beneficiary’s post or bank account.

This application form can be obtained free of charge from the Mamlatdar office.
Application can be done online from Village Level (V.C.E.) Gram Panchayat.
