How to Apply for PM Jay G Card 2025?

Applying for the PM Jay Card 2025 is straightforward and can be done through multiple channels:

1️⃣ Online Application Process:

Visit the official portal: https://pmjay.gov.in/
Click on “Am I Eligible” to check your eligibility by entering your details.
If eligible, proceed to fill out the application form with your Aadhaar card, family details, and income information.
Upload necessary documents: Identity proof, Address proof, and Income certificate.

1. Certificate Collection

Fill up the Certificate Format decided by your department.
Get it signed & sealed by Head of Department.

2. Submit Certificate

Submit the certificate to your DDO (Drawing & Disbursing Officer) or PMJAY Nodal Officer.
They will forward the details to SHA (State Health Agency), Gujarat.

3. E-KYC Verification

Do Aadhaar-based e-KYC for you & dependents.
Can be done Online or through Camp.

4. G Card Generation

After successful verification, PMJAY G Card will be generated.

Get Certificate from District Treasury / Sub-Treasury / Pension Payment Office.

નિવૃત્ત કર્મચારીઓ (પેન્શનરો) માટે કાર્ડ મેળવવાની પ્રક્રિયા:

1. પ્રમાણપત્ર મેળવો

નજીકની જિલ્લા ટ્રેઝરી, સબ-ટ્રેઝરી, અથવા પેન્શન પેમેન્ટ ઓફિસ પર જઈને સહી કરાવેલું પ્રમાણપત્ર મેળવો.
તમે નિવૃત્ત થયેલા વિભાગના વડા પણ આ પ્રમાણપત્ર આપી શકે છે.

2. SHA ગુજરાતને સબમિટ

સંબંધિત અધિકારી દ્વારા પ્રમાણપત્ર SHA Gujarat પાસે મોકલવામાં આવશે.

3. E-KYC કરાવવી ફરજિયાત

તમારા તેમજ આશ્રિતો માટે આધાર આધારિત E-KYC કરાવવી ફરજિયાત છે.

4. G કાર્ડ generat થશે

સફળ ચકાસણી બાદ તમારું PMJAY G Card જનરેટ થશે.
